Role overview

Responsibilities - Take care‑management responsibility for vulnerable clients, including production of a comprehensive social assessment of client needs, formulation of a social care plan and identification of relevant resources to meet the individual’s needs. When appropriate, following assessment, co‑ordinate the delivery of social care plans and liaise with the appropriate government or charity agencies. Work alongside and in partnership with individuals, families and caregivers using a range of interpersonal and communication skills to assess highly complex psycho‑social needs, set objectives, develop care plans and deliver appropriate interventions. Assist the multidisciplinary team to recognise and intervene as necessary for high‑risk patients using the approved Social Work Criteria for the Assessment of Patients with Social Issues. Actively work with Nurse Case Coordinators to plan and intervene for discharge planning, liaising with support agencies such as Social Support Centres, Embassies and Charitable Foundations. Conduct initial comprehensive person‑centred assessments, plan and implement individual strategies of care to meet identified needs in conjunction with the service user, caregiver and members of the multidisciplinary team. Support individuals, families and caregivers through loss, change, illness and uncertainty; act as an advocate for the patient, ensuring that their expressed wishes are central when planning care to meet the client’s long‑term needs. Undertake comprehensive interdisciplinary assessments, early intervention, safe and effective decision‑making to assist the care team, overcome discharge barriers in a timely manner and facilitate the transition of the patient back to the community and an appropriate care setting. Communicate with patients and other members of the team to assist the patient’s progress along the continuum of care towards return to wellness; advocate for patients who may have difficulty understanding or communicating. Respect the rights of the individual service user and their families, embracing their values, spiritual beliefs, customs and social norms in daily activities. Promote awareness of social care and social work context, anti‑discriminatory practice, and facilitate multidisciplinary teamwork. Enable and empower individuals, families and caregivers through representation of rights, advocacy, nurturing strengths and capacities to make informed choices about highly complex life decisions. Network and liaise with other professionals and external organisations to provide services to individuals, families and carers, and challenge and question when necessary in the best interests of the user. Communicate information and advice to service users, caregivers, families, agencies, providers and professional colleagues verbally and in written form with patient consent when appropriate. Write letters, reports and other documents required for individual patient needs, including Social Study Reports, letters to charitable foundations, embassies, Sheikh’s offices, social support services and any other social organisation as required. Maintain and promulgate the highest standards of clinical record‑keeping, including electronic data entry, reporting and the responsible exercise of professional self‑governance in accordance with professional codes of practice. Maintain confidentiality of information about patients, staff and others in accordance with professional codes of conduct and relevant legislation; document in the patient medical record following every encounter or intervention. Ensure compliance with all safety and quality control programmes and procedures as applicable; maintain and assist in enforcing the strictest confidence in relation to patient information, both formally and informally recorded. Assist in the planning and development of policies that guide the service, practising within the expressed limits of established policies and procedures. Deliver and participate in training and education sessions for department staff and multidisciplinary team relating to the social work role and activities. Participate in department program committees, various hospital committees and staff meetings. Participate in orientation and training of new employees; ensure appropriate internal and external sharing of information based on governing policies and procedures. Maintain professional knowledge by attending lectures, seminars or online education units; complete and maintain all regulatory requirements, including licensure, certification and other mandatory training within established time frames. Ensure all members of the department receive adequate clinical supervision as required for their professional and technical development. Qualifications - Bachelor’s Degree in Social Work. Master’s Degree in Social Work. UAE Nationals. Clinical Social Work License.
